Sick Leave Application for College: Free Template + Medical Certificate Guide

Most students struggle to write a professional sick leave application for college or a medical leave application for college when they fall ill. Finding the right format, tone, and knowing what documents to attach can be confusing. In this guide, we provide a free application template that you can easily copy and modify, along with guidelines on attaching a medical certificate to ensure your leave is approved by the university HOD or coordinator.

The template

To, The Class Coordinator / HOD, [Department Name], [College Name] Subject: Application for Sick Leave Respected Sir/Madam, I am [Your Name], a student of [Course/Year/Section], Roll No. [Roll Number]. I am writing to inform you that I have been unwell since [start date] and am unable to attend classes. I am attaching a medical certificate from [Doctor's Name / Clinic] confirming the same. I kindly request you to grant me leave from [start date] to [end date] and consider my attendance shortage, if any, for this period. Thank you for your understanding. Yours sincerely, [Your Name] [Roll Number] [Contact Number] [Date]

Simple, direct, and it puts the two things that actually matter — dates and proof — right up front instead of burying them in paragraph three.

Why the application alone often isn't enough

A lot of students submit the application and assume that's the end of it. For a genuinely short absence, that's often fine. But once you're talking about more than a day or two, or your attendance is already tight, an application without a certificate reads as an unverified excuse, not documented illness — and departments increasingly want the certificate specifically because applications alone are easy to write regardless of whether you were actually unwell.

Where to send it, and who actually needs to see it

This varies by college, but the general pattern is:

- Short, routine absence: class coordinator or subject teacher is usually enough

- Longer absence, or if attendance is already a concern: HOD or department office, sometimes with a copy to the exam section

- Hostel students: the warden may also need a copy, separate from the academic side

If you're not sure, sending it to your coordinator first and asking them where else it needs to go is a completely normal question — nobody expects first-years especially to know the full escalation chain.

Pairing the application with a real certificate

The application explains what happened; the certificate proves it. If you don't want to lose a day tracking down a clinic while unwell, a quick teleconsultation gets you a signed, doctor-issued certificate you can attach directly to the application above. On Doctor Certificate, this starts at ₹349 and is typically ready within 30 minutes.

Frequently asked questions

Is the application enough on its own?

For a short 1-2 day absence, sometimes. For longer absences or tight attendance situations, a doctor's certificate is usually expected alongside it.

Who do I address it to?

Usually your class coordinator or HOD, depending on the college's structure and how serious the absence is.

How to write sick leave application for college?

College ke liye sick leave application likhne ke liye aapko to-the-point format follow karna chahiye. Sabse pehle Class Coordinator ya HOD ko address karein. Subject line mein clear likhein. Paragraph 1 mein apna roll number, name aur class mention karein. Paragraph 2 mein leave ki exact dates aur bimaari ka brief detail dekar medical certificate attach karein.

What to write in medical leave application for college?

Medical leave application mein aapko apna legal name, roll number, department name aur exact dates of leave specify karni hoti hain. Iske sath hi leave ka clinically valid medical reason jaise fever or recovery mention karein aur supporting doctor-signed medical certificate photocopy attach karke submit karein.

Is medical certificate required for 1 day college leave?

Generally, 1-day leave ke liye sirf written sick leave application coordinator ko submit karna kafi hota hai. Par agar aapki attendance percentage critical (75% se kam) hai, ya exams miss ho rahe hain, toh strict college guidelines ke under 1-day absence ke liye bhi doctor-signed medical certificate mandatory ho jata hai.
