Most corporate HR policies in India require a formal medical certificate for any sick leave exceeding two or three consecutive days. To prevent delays or rejection, your medical certificate must be structured professionally.

Must-Have Elements on a Sick Leave Certificate

Ensure your certificate includes these key components before submitting it to your HR portal:

- Patient Identification: Your full name, age, gender, and contact details matching your employee profile.

- Specific Fit/Unfit Dates: The exact start and end dates of the recommended rest period.

- Registered Doctor Credentials: The full name, degree, and active registration number of the certifying doctor.

- Verification System: A unique Certificate ID that HR can verify independently to rule out forgery.
